Objectives
OBJECTIVES: Upon completion of this course, the participant will be able to:
1. Discuss the social, psychological and physical health implications of complex body shapes.
2. Describe how body position, soft tissue adaptation, gravity and time affect the ribcage, spine, pelvis and hips.
3. Demonstrate understanding of how to conduct a mat evaluation to analyze postural problems.
4. Predict the future likely direction of chest rotation, pelvic distortion and hip dislocation.
5. Distinguish between therapeutic and destructive postures in lying and sitting.
6. Describe the impact of sleep patterns, thermal regulation, and behavior on posture support interventions.
7. Identify team roles and potential safety risks when creating posture management plans
8. Demonstrate ability to identify and utilize appropriate outcome measure
9. Use information gained from assessments to create a therapeutic positioning plan.
10. Demonstrate low-tech ways to improve postural support in lying, seating systems and household furniture.

Day 1
8-9 am – Background, social, psychological and health implications of distorted body shapes. Prevention and possible restoration of distorted body shapes. 9-10 am - How body position, soft tissue adaptation, gravity and time affect the ribcage, spine, pelvis and hips. 10:15 – 12:15 pm - Predict the likely direction of chest rotation, pelvic distortion and hip dislocation based on habitual lying postures. Understand three sitting postural tendencies, contrasting destructive and positive aspects and link with lying. 1-2 pm - Mat evaluation demo 2-3:30 pm - Mat evaluation group practice 3:45 – 5:30 pm – More practice and group presentations/discussion of mat evaluation analysis of postural problems
Day 2
8:00 – 8:30 – Questions/discussion from Day 1 8:30 – 10:30 - Outcome measures - demonstration/lab 10:45 – 12:15 - Sleep patterns, thermal regulation, team roles, supporting behavior change and risks vs benefits. 1:15 – 2:15 Low-tech ways to improve postural support in lying and seating systems or household furniture. 2:30 – 4:30 Lab - Implement low tech therapeutic sleep positioning and seating support simulation 4:30 – 5:00 Presentations/discussion about lab experience 5:00 – 5:30 Questions and Adjourn

Description
The bodies of children with chronic motor impairments often distort as they grow, developing joint contractures and dislocations, scoliosis and kyphosis. The problems start early – often related to habitual postures assumed both day and night – and can worsen quickly during growth spurts. Human beings spend their lives in three orientations - lying, sitting and standing - and for children with limited movement this can become complicated! Too often, the foundational orientation of lying is overlooked, as we work toward upright orientation goals. 24-hour posture care management (24-7 PCM) analyzes and addresses postures that can become destructive over time. Non-invasive strategies are used to protect growing bodies from distorting, in some cases restoring them to better alignment without surgery.
In the United States this approach is novel but becoming recognized for its potential to protect and improve body alignment, support restful sleep, safeguard respiratory and digestive functions and reduce pain. 24-7 PCM builds a foundation of body symmetry and stable posture that will support therapeutic goals in all areas of development. 24-7 PCM promotes the support of healthy posture in all orientations as crucial for children with limited movement repertoire who cannot change their positions easily, often and with variety. Lying postures with poor body alignment may be noted, but are often not routinely addressed in treatment planning – to the detriment of growing kids.
This course provides an evidence based overview of theory and practice with visual examples, longitudinal case studies and hands-on practice to illustrate concepts underpinning 24-7 PCM. The importance of early therapeutic positioning, seating and lying assessments, learning to predict likely distortions early on and how to effectively create and implement 24-7 PCM plans is emphasized.
