Objectives
Upon completion, participants will be able to:
1. Compare and contrast ways to creatively progress sit to stand movement task/training and educate patients and caregivers regarding the concepts of base of support and center of gravity to increase independence with transfers.
2. Identify two assessments that can be used as fall risk assessments and indicate cutoff scores for fall risk in the identified assessments.
3. Explain the rationale for utilizing hand-held dynamometry over manual muscle testing.
4. Compare methods of strength training in older persons.
5. Delineate the steps in calculating 1RM using the Borg scale.
6. Discuss treatment progression and monitoring for functional training of a hip fracture patient.

Agenda
Part 1 (90 minutes) The therapist assesses patient initially in the patient’s room, takes a comprehensive medical history and establishes baseline measurements. Then the combination evaluation and first day treatment will continue in the gym with calculation of a 1 RM and a type 1 and type 2 strength training program. Functional sit to standing, balance progression and measurement are demonstrated as well as other therapeutic exercises, activities and neuromuscular re-education.
Part 2 (60 minutes) The second visit will include further testing deemed appropriate and necessary, as well as the progression of some of the exercises presented day one. Stair climbing and balance deficit training will be shown and progressed, in addition to trunk stabilization and home program suggestions.
Part 3 (2.5 hours) Explore clinical decision making with the master clinician as she responds to questions and comments led by Dr. Carole Lewis, PT with the help of Keith Screen, OTR. Enjoy listening to high level collegial discussion loaded with evidence based practice references and clinical suggestions.

Watch 5 hours of live video demonstrating cutting edge practice. Spend 2.5 hours with an experienced clinician in a skilled nursing facility as she meets for the first time and then treats an86-year old woman over a 2-day period. Carolyn Padalino takes a patient, who has multiple diagnoses, including hip fracture, through initial evaluation and two treatment sessions. Watch closely as she works with a real patient to demonstrate patient intake, exercise programming, progression and patient monitoring. In the second half of the course, you will review the evaluation and treatment sessions while Carolyn explains her clinical decision-making and expands on why the research-based interventions she chose were the most appropriate. This commentary is led by Carole B. Lewis, PT, DPT, GCS, GTC, CCOEE, MPA, MSG, PhD, FAPTA with input from Keith Screen, MS, OTR. The discussion is packed with evidence based suggestions and support. You will learn numerous treatment strategies and new ideas for care that you can apply in your clinic immediately
